Cosmetic consultations are complimentary.  Appointments are available
every weekday, depending upon appointment availability. Medical
consultations are billed to a patient's insurance company.  We  participate
with most major medical insurers.

Please print and complete both our Patient Medical History Form and our
Patient Profile Form and bring each with you to your consultation.  You will
need to download the free version of Adobe Acrobat Reader to print our forms.
